CO-4 denial: meaning, fix & appeal letter template

Modifier missing, invalid, or inconsistent with the procedure.

What it means

Official CARC 4 descriptionThe procedure code is inconsistent with the modifier used.

The payer rejected the line because the modifier on it doesn't fit the CPT/HCPCS code — or a modifier the payer requires (laterality, professional/technical component, distinct service) is missing.

Common causes

  • Required modifier omitted (e.g. -26/-TC on split-billed radiology, -RT/-LT for laterality)
  • Modifier not valid for that code (e.g. -25 appended to a procedure instead of the E/M)
  • Conflicting modifiers on the same line (e.g. -26 and -TC together)
  • Payer-specific modifier rules (e.g. -GP/-GO/-GN for therapy, -XU/-XS instead of -59)
  • Modifier keyed in the wrong position or dropped by the clearinghouse

How to fix it

  1. Pull the 835 and confirm which line and modifier triggered the denial (check RARC remarks)
  2. Compare the code against CPT/HCPCS modifier guidance and the payer's modifier policy
  3. Confirm documentation supports the corrected modifier
  4. Submit a corrected claim (frequency code 7) with the right modifier
  5. Add a scrubber edit so the same code/modifier combination is caught before submission

Appeal or corrected claim?

Usually a corrected claim, not an appeal. CO-4 is a billing error, so most payers want a replacement claim (frequency code 7, with the original claim number). Appeal only if the modifier was correct as billed and the payer applied its edit wrongly — then cite the payer's own modifier policy.
