CO-16 denial: meaning, fix & appeal letter template
Missing or invalid data on the claim — usually fixable by resubmission.
What it means
Official CARC 16 descriptionClaim/service lacks information or has submission/billing error(s).
The payer couldn't adjudicate the claim because something is missing or invalid. CO-16 almost always comes with a RARC (remark code, e.g. M51, N290, MA130) that tells you exactly which field.
Common causes
- Missing or invalid referring/rendering NPI
- Missing place of service or invalid POS for the code
- Subscriber ID, name, or date of birth mismatch
- Missing NDC for drug codes, or missing units
- Missing attachment or claim note the payer requested
How to fix it
- Read the RARC on the 835 — it names the missing or invalid element
- Correct the field in the practice-management system
- Resubmit as a corrected claim (frequency code 7) with the original claim number
- Track the RARC trend — repeat CO-16s point to a registration or charge-entry fix
Appeal or corrected claim?
Corrected claim, not an appeal. CO-16 denials generally carry no appeal rights because the claim wasn't processable as submitted. Fix the data and resubmit. Use a reconsideration letter only if the information was present on the original claim and the payer missed it.
What to attach
- Corrected claim (frequency code 7) referencing the original claim number
- Requested documentation, if the RARC asks for an attachment
- Clearinghouse report showing original data, if the payer dropped it
